กรณีพบเห็นโดยเจ้าหน้าที่ความปลอดภัยวิชาชีพคณะกรรมการความปลอดภัยหรือผู้จัดการฝ่าย ให้บันทึกรายละเอียดการกระทำที่ผิด ( ใคร อะไร อย่างไร ที่ไหน และ เมื่อไหร่ ) ลงในแบบฟอร์ม แล้วส่งผู้เกี่ยวข้องตามขั้นตอน ( หน่วยงานเซฟตี้จะเป็นผู้ประสานงานการลบคะแนนต่อไป)
