สถาบันพยาธิวิทยา กรมการแพทย์ เป็นหน่วยงานระดับกอง สังกัดกรมการแพทย์ กระทรวง สาธารณสุข ได้ถือกำเนิดขึ้นเมื่อวันที่ 10 ธันวาคม 2517 ตามประกาศในราชกิจจานุเบกษา เล่ม 91 ตอนที่ 210 ในช่วงที่ ฯพณฯ ศาสตราจารย์นายแพทย์อุดม โปษะกฤษณะ เป็นรัฐมนตรีว่าการกระทรวงสาธารณสุข และมี ฯพณฯ ศาสตราจารย์นายแพทย์เสม พริ้งพวงแก้ว เป็นรัฐมนตรีช่วยว่าการกระทรวงสาธารณสุข ซึ่งคณะผู้บริหารของกระทรวงสาธารณสุขในขณะนั้น ได้มองการสาธารณสุขของประเทศในระยะยาว และตระหนักถึงความสำคัญของพยาธิวิทยา โดยเฉพาะความจำเป็นของวิทยาการสาขานี้ที่มีต่อการบำบัดรักษาโรค สามารถจะดำเนินการรักษาได้อย่างมีประสิทธิภาพและเป็นประโยชน์โดยตรงต่อประชาชนผู้เจ็บป่วย

อัตรากำลัง ในระยะแรกมีเพียง 22 คน เป็นพยาธิแพทย์ 7 คน และเจ้าหน้าที่อื่น ๆ 15 คน ซึ่งทั้งหมาดนี้ได้ย้ายมาจากแผนกพยาธิวิทยาของโรงพยาบาลหญิง หรือโรงพยาบาลราชวิถีในปัจจุบัน โดยมี พ.ญ. นวรัตน์ ณ สงขลา เป็นผู้อำนวยการสถาบันฯ สภาพการทำงานในระยะนั้นเป็นการให้บริการด้านพยาธิวิทยาแก่หน่วยงานต่าง ๆ ในสังกัดกระทรวงสาธารณสุข โดยมีลักษณะงานเป็นหน่วยงานบริการกลาง แต่งานส่วนใหญ่ได้มาจากโรงพยาบาลหญิง การเงินงบประมาณ จึงยังคงแฝงอยู่ในงบประมาณของโรงพยาบาลนั้น จนถึงปีงบประมาณ 2519 สถาบันพยาธิวิทยา จึงได้รับงบประมาณเป็นของตนเอง การบริหารและ การจัดการต่าง ๆ มีความคล่องตัวมากขึ้น พร้อมทั้งภารกิจที่ได้รับมอบหมายจากกรมการแพทย์ กระทรวง สาธารณสุขก็เพิ่มขึ้นด้วย กล่าวคือ รับผิดชอบในการฝึกอบรมแพทย์ประจำบ้าน สาขาพยาธิวิทยา หลักสูตร 3 ปี ตามมาตรฐานที่แพทยสภากำหนด (ต่อมาได้รับอนุมัติเพิ่มเติมเป็นสาขาพยาธิวิทยากายวิภาค และสาขาพยาธิวิทยาทั่วไป) จากปีงบประมาณ 2519 ถึงปัจจุบัน สถาบันฯ ได้ผลิตพยาธิแพทย์ให้แก่ส่วนราชการไปแล้วจำนวน 19 คน (ตั้งแต่ปี 2537 เป็นต้นมา ไม่มีผู้สมัครเข้าอบรม)


ปีงบประมาณ 2520 ได้เริ่มจัดหลักสูตรฝึกอบรมแพทย์ สาขาพยาธิวิทยา หลักสูตร 1 ปี และหลักสูตรฝึกอบรมเจ้าหน้าที่วิทยาศาสตร์การแพทย์ สาขาพยาธิวิทยา โดยจัดตั้งเป็นโรงเรียนพนักงานวิทยาศาสตร์- การแพทย์ สาขาพยาธิวิทยา หลักสูตร 1 ปี ทั้งนี้ เพื่อ ส่งไปปฏิบัติงานในโรงพยาบาลในส่วนภูมิภาค เป็นการแก้ปัญหาเฉพาะหน้าของประเทศเกี่ยวกับการขาดแคลนบุคลากรทางพยาธิวิทยา การอบรมทั้ง 2 หลักสูตรได้เริ่มขึ้นครั้งแรกปี 2521 สถาบันพยาธิวิทยาได้ผลิตแพทย์ สาขาพยาธิวิทยาหลักสูตร 1 ปี จำนวน 1 คน ส่วนหลักสูตรเจ้าหน้าที่วิทยาศาสตร์การแพทย์ สาขาพยาธิวิทยา จำนวน 17 คน (หลักสูตรเดิมและใหม่) จนถึงปี 2537 มีผู้สำเร็จการศึกษารวม 273 คน

ปีงบประมาณ 2524 ได้ขยายงาน โดยการจัดตั้งโรงเรียนเวชสาธิตหลักสูตร 1 ปีขึ้น โดยรับนักเรียน ผู้จบ ม.ศ. 5 หรือ ม.6 หรือ ปวช. ด้านศิลปกรรม เข้าศึกษาจนถึงปี 2537 มีผู้สำเร็จการศึกษาจำนวน 13 รุ่น รวม 265 คน และปรับปรุงหลักสูตรวิชาเจ้าหน้าที่วิทยาศาสตร์การแพทย์ สาขาพยาธิวิทยา ให้เหมาะสมยิ่งขึ้น โดยกำหนดความรู้พื้นฐานของผู้เข้าเรียนเพิ่มจากเดิมวุฒิ ม.ศ. 3 เป็น ม.ศ. 5 หรือ ม.6 เพื่อให้ผู้เข้ารับการศึกษามีขีดความสามารถ และหน้าที่รับผิดชอบสูงขึ้น และในปี 2538 สำนักงานปลัดกระทรวงสาธารณสุขได้รับทั้ง 2 หลักสูตรไปดำเนินการเอง สถาบันฯ จึงรับผิดชอบสนับสนุนด้านการสอนภาคทฤษฎีและปฏิบัติเพียง อย่างเดียว

ปีงบประมาณ 2529 ได้ริเริ่มจัดทำโครงการฝึกอบรมระยะสั้น หลักสูตรการพัฒนาเจ้าหน้าที่โสตทัศน-ศึกษา, หลักสูตรการฟื้นฟูวิชาการสำหรับเจ้าหน้าที่วิทยาศาสตร์การแพทย์ สาขาพยาธิวิทยา และจัดการประชุมวิชาการสำหรับพยาธิแพทย์สังกัดกระทรวงสาธารณสุข
