วันที่ 25/09/2016 มีคำสั่งจาก TPC ให้ทำการส่ง VCM จาก Tank O-FA811C to TPC จำนวน 300 ton
จึงได้สั่งให้ Field operator ออกไปทำการจัด Line VCM เพื่อทำการส่ง และได้เริ่มทำการส่งเวลา 21.52 น.
และได้พบเห็นว่ามีการเพิ่มขึ้นที่ Tank O-FA811B เกิดขึ้น เวลา 23.10 น.จึงได้ทำการหยุดส่งชั่วคราวเพื่อตรวจสอบหาสาเหตุโดยพบว่ามี Vale no. 9 ของ Tank O-FA811B ปิดไม่สนิทอยู่ 1 ตัว โดยที่ Valve เปิดอยู่เล็กน้อย และ
ได้ทำให้ VCM passing เข้าไปจำนวน 83 ton และได้แจ้งทาง TPC รับทราบเวลา 23.15 น.ไว้เบื้องต้นแล้ว โดยที่มี VCM ส่งไปทาง TPC แล้ว 217 ton จึงยังขาดอยู่ตามจำนวนที่เข้า Tank O-FA811B และทาง TPC ได้แจ้งกับมาว่า
ให้ทำการส่งให้ครบ 300 ton และ ได้ขอเพิ่มขึ้นอีก 100 ton เมื่อทำการตรวจสอบและทำการแก้ไขที่ Valve no.9 และทุกตัวเรียบร้อยแล้ว จึงได้แจ้ง TPC อีกครั้งว่าเวลา 23.30 น.จะทำการเริ่มส่ง VCM ให้กับทาง TPC ตามจำนวน
ต่อไปจนครบ
ณ.ปัญหาที่เกิดขึ้นนี้จึงได้กำชับกับ Fill operator ให้เป็นแนวทางครั้งต่อไปว่า ถ้ามีการ Move ต่างๆให้ทำการตรวจสอบ Valve ที่เกี่ยวข้องทุกตัวเสมอก่อนทำการส่งทุกๆครั้ง
