1.1 เกิดจากความผิดพลาดของการบริหารจัดการซึ่งมีรายระเอียดปลีกย่อยต่างกันดังนี้
1) ไม่มีการสอนหรืออบรมเกี่ยวกับความปลอดภัย
2) ไม่มีการบังคับให้ปฏิบัติตามกฎหมายความปลอดภัย
3) ไม่มีการวางแผนและเตรียมงานด้านความปลอดภัย
4) ไม่มีการแก้จุดอันตรายต่าง ๆ
5) ไม่มีการจัดหาอุปกรณ์ป้องกันอันตรายให้
1.2 สภาวะของคนงานไม่เหมาะสม กล่าวคือ
1) ขาดความระมัดระวัง
2) การทำงานของสมองไม่ประสานกัน
