ขั้นที่ 1 ประธานมูลนิธิหรือกรรมการมูลนิธิ ผู้ได้รับมอบหมายยื่นเรื่องราว (แบบ ม.น.2) ที่สํานักงานใหญ่ของมูลนิธิตั้งอยู่ (สํานักงานเขตในกรุงเทพมหานครและที่ทําการปกครองอําเภอ) พร้อมทั้งหลักฐานจํานวน 3 ชุด (เอกสารทุกอย่างต้องรับรองสําเนา) คือ

1. แบบ ม.น.2
2. บัญชีงบดุลซึ่งผู้สอบบัญชีรับอนุญาตได้ลงนามรับรองแล้ว
3. บันทึกการประชุมคณะกรรมการมูลนิธิ ซึ่งมีมติให้เปลี่ยนแปลง
4. ข้อบังคับมูลนิธิ
5. บัญชีรายชื่อกรรมการชุดเก่า-ใหม่
6. บันทึกการสอบสวนคุณสมบัติกรรมการ (เฉพาะบุคคลที่ไม่เคยเป็นกรรมการมูลนิธิใดมาก่อน)
7. กรณีกรรมการเป็นชาวต่างชาติ ให้ดําเนินการตามแนวทางหนังสือกรมการปกครอง ที่ มท 0307.4/ว 4107 ลว. 3 มีนาคม 2549
