สัญญาฉบับนี้จัดทำขึ้นระหว่างบริษัท ทริม อินเตอร์เนชั่นแนล จำกัด ตั้งอยู่ที่ 1342 หมู่ที่ 1 ถนนราชสีมา-โชคชัย

ข้อ 2. ผู้ว่าจ้างตกลงจะจ่ายเงินค่าจ้างให้กับผู้รับจ้าง เป็นจำนวนเงิน........................บาท(.....................)

เวลาปฏิบัติงาน 07.01-16.00 น. จำนวน 2 คน
16.01-23.00 น. จำนวน 2 คน
23.01-07.00 น. จำนวน 2 คน
และมีพนักงานเพิ่มอีก จำนวน 1 คน เพื่อสับเปลี่ยนหมุนเวียนการทำงาน
รวมจำนวน 7 คน
ซึ่งพนักงานแต่คนจะต้องทำงานรวมทั้งสิ้นสัปดาห์ละไม่เกิน 60 ชั่วโมง และใน 1 สัปดาห์ ต้องจัดวันหยุดให้อย่างน้อย 1 วัน

ข้อ 3. ผู้รับจ้างสัญญาว่าจะจ้างพนักงานรักษาความปลอดภัย โดยมีค่าจ้างขั้นต่ำให้เป็นไปตามประกาศของกระทรวงแรงงานและจัดให้มีสวัสดิการประกันสังคม และ ส่งเงินสมทบเข้ากองทุนทดแทนประกันสังคม /หรือ ทำประกันสุขภาพ ของพนักงานผู้รับจ้าง เพื่อเป็นหลักประกันในการดูแลรักษาสุขภาพ หรือ อุบัติเหตุ ที่อาจเกิดขึ้นในอนาคตภายหน้า และให้ผู้ว่าจ้างสามารถเรียกตรวจเอกสารตามที่ได้กล่าวมานี้ได้

ข้อ 4. สัญญานี้กำหนดเวลาไว้ จำนวน 1 ปี นับแต่วันที่เริ่มทำสัญญา และเมื่อสัญญาครบกำหนดฝ่ายหนึ่งฝ่ายใดไม่ได้แจ้งยกเลิกให้ถือว่าสัญญานี้ต่อไปอีกคราวละ 1 ปี เว้นแต่หากฝ่ายใดฝ่ายหนึ่งมีความประสงค์ที่จะยกเลิกสัญญาจ้างนี้ ให้แจ้งยกเลิกเป็นหนังสือให้กับอีกฝ่ายหนึ่งอย่างน้อย 30 วัน

ข้อ 5. ผู้ว่าจ้าง ตกลงว่าจะแจ้งให้ผู้รับจ้างทราบทันที ถึงเหตุการณ์ที่มีผลกระทบต่อความปลอดภัยของสถานที่ ทรัพย์สิน ที่ได้รับความคุ้มครองความปลอดภัย รวมถึงคำร้องทุกข์ หรือข้อเสนอแนะใด ๆ เกี่ยวกับการทำงานของพนักงานรักษาความปลอดภัย

ข้อ 6. ผู้รับจ้างตกลงจะปฏิบัติดังต่อไปนี้
